Information For Patients
Patients are appointed on a referral basis only - a patient must be under the care of a doctor or dentist who orders a specific imaging examination for a particular diagnostic purpose. Patients must present to the office with a radiographic prescription or “letter” provided by their referring practitioner or services may be delayed.

Appointment Details
Appointments
Appointments can be made by calling 502-852-1243. You will contact one of our medical certified Radiologic Technologists (Barbara or Elaine). They will be able to answer all your questions about the procedure and welcome you to the practice.
Location
Our CBCT imaging services are provided in the University of Louisville School of Dentistry, 501 South Preston Street, Louisville KY USA
This is designated as C on the Map of the Health Sciences Center (below)

Directions
You can obtain driving directions from your home or office to our facility from MapQuest.
Hours
Patients are seen only by appointment, Monday through Friday from 9 AM to 3:15 PM. Later hours can be made by appointment only.
Billing Policy
Patients are responsible for fees at the time of their appointment. Visa, MasterCard, cash and personal checks are accepted. Appropriate dental or medical insurance forms will be completed and filed on your behalf.
Images
As the i-CAT™ images are inherently digital, original data is held by our office. Selected images and a comprehensive written report are mailed (either electronically or via "snail mail") to the referring practitioner within 2 days. If reports and images are urgently required, please inform the OMF radiologists at the time of your arrival.
Reports
A written report is always provided and will be forwarded with the selected images.
